How does hydrochlorothiazide + lisinopril work?
Lisinopril reduces angiotensin II and aldosterone, helping blood vessels relax. Hydrochlorothiazide increases the kidneys’ removal of sodium and water. Together, these effects lower blood pressure.
How to take hydrochlorothiazide + lisinopril
Follow the prescription label and swallow the tablet with water. It may be taken with or without food. The score line is not designed for dividing the tablet.
Dizziness can occur, particularly when treatment changes. Avoid driving or using machinery until you know how you are affected.

- Severe breathlessness or fluid or inflammation in the lungs
- Kidney failure or passing little or no urine
- Severe electrolyte disturbance, possibly causing weakness, confusion, cramps or an abnormal heartbeat
- Sudden painful red eye or blurred vision
- Severe skin blistering or peeling
- Jaundice or other signs of severe liver injury
hydrochlorothiazide + lisinopril in pregnancy
This combination is not recommended in early pregnancy and is contraindicated during the second and third trimesters. Anyone planning pregnancy or who becomes pregnant should promptly contact their prescriber to arrange an alternative. Exposure after about 20 weeks requires specialist assessment and additional fetal monitoring.
hydrochlorothiazide + lisinopril while breastfeeding
The combination is not recommended while breastfeeding, particularly for a newborn or premature infant. Evidence is limited, and thiazides may reduce milk production. An alternative with better-established evidence is generally preferred.
