Treating Paget’s disease of bone.
How does pamidronate disodium work?
Pamidronate binds strongly to bone mineral, particularly where bone turnover is high. It suppresses osteoclast-mediated bone resorption, helping lower calcium released into the blood and reduce bone damage associated with certain cancers or Paget’s disease.
How to take pamidronate disodium
Pamidronate disodium is prepared and administered in hospital or another supervised clinical setting.
It is diluted before use and given as a slow intravenous infusion, never as a bolus injection.
Follow the clinical team’s hydration instructions.
Maintain good oral hygiene, attend routine dental checks and tell your dentist that you receive pamidronate.
Arrange transport home after an outpatient infusion.
pamidronate disodium side effects
Common or expected effects
- Flu-like symptoms, fever, chills or tiredness, particularly after the first infusion
- Low blood calcium or phosphate, often detected on blood tests
- Pain, redness, swelling or inflammation around the infusion site
- Headache, drowsiness or tingling sensations
- Nausea, vomiting, abdominal discomfort, diarrhoea or constipation
- Temporary bone, muscle or joint pain
- Rash
- Changes in blood-cell counts or kidney blood-test results
Get urgent medical advice
- Severe allergic reaction, including facial or throat swelling, breathing difficulty or collapse
- Symptomatic low calcium, such as marked tingling, muscle spasms or seizures
- Kidney injury, which may present with substantially reduced urine or swelling
- Eye inflammation causing pain, redness or visual disturbance
- Jaw pain, swelling, loose teeth, discharge or sores that do not heal, suggesting osteonecrosis of the jaw
- New thigh, hip or groin pain, which may precede an unusual femoral fracture
- Severe breathing difficulty or heart-failure symptoms
- New irregular heartbeat
pamidronate disodium in pregnancy
Pamidronate is not usually recommended during pregnancy because human safety evidence is limited and bisphosphonates remain stored in bone. The selected product restricts use in pregnancy to life-threatening hypercalcaemia that cannot be controlled by other means. Discuss a planned or new pregnancy promptly with a specialist.
pamidronate disodium while breastfeeding
Breastfeeding during pamidronate therapy is not recommended. Experience is extremely limited and an effect on bone mineralisation cannot be excluded; specialist advice is required.
pamidronate disodium interactions
The treating team should review all prescribed, over-the-counter and complementary products before administration.
Other bisphosphonates
Do not use concurrently because combined effects have not been adequately investigated.
Potentially nephrotoxic medicines
May increase the risk of kidney impairment; caution and renal monitoring are required.
Anti-angiogenic medicines
Concurrent treatment has been associated with a higher incidence of osteonecrosis of the jaw.
