This patient has the specific NICE phenotype for specialist-initiated roflumilast: severe airflow obstruction defined by post-bronchodilator FEV₁ below 50% predicted, chronic bronchitis and at least two exacerbations in the previous 12 months despite LAMA/LABA/ICS triple therapy. He has no identified contraindication: liver function is normal, weight is satisfactory and there is no relevant psychiatric history. The current SmPC recommends 250 micrograms daily for 28 days to improve initial tolerability, followed by the therapeutic maintenance dose of 500 micrograms daily. Azithromycin is an attractive exacerbation-prevention option because he produces sputum and has had frequent and hospital-treated exacerbations. However, NICE recommends considering it in people who do not smoke and requires exclusion of a prolonged QT interval; this patient fails both discriminators. Theophylline is less effective and less well tolerated than inhaled long-acting bronchodilators and is generally reserved for patients unable to use inhaled therapy or still symptomatic after inhaled bronchodilator trials; it is not the preferred phenotype-directed add-on here. Maintenance oral corticosteroids should normally be avoided in stable COPD because of systemic toxicity. Carbocisteine can be trialled for chronic productive cough and continued if symptoms improve, but it does not supersede the directly applicable NICE recommendation for roflumilast in this frequent-exacerbator chronic-bronchitis phenotype.
