Correct answer: ATreat with a targeted oral antibiotic and continue COPD exacerbation care

Moraxella catarrhalis is a recognised COPD exacerbation and pneumonia pathogen, and targeted antibiotic therapy is appropriate when consistent with clinical infection. Aspergillosis and TB are not supported by the acute culture and radiographic context. Bronchodilators remain part of COPD care, and ignoring the isolate would be unsafe.
