OVHIPEC-1 (van Driel et al NEJM 2018) randomised stage III EOC patients to interval CRS alone vs CRS + HIPEC (cisplatin 100 mg/m² at 40°C for 90 minutes) after 3 cycles of NACT. RFS improved (14.2 vs 10.7 months) and OS improved (45.7 vs 33.9 months). However, results are debated: single-centre trial, open-label, and OVHIPEC-2 (evaluating in front-line) is ongoing. Current guidelines consider HIPEC in interval debulking setting at experienced centres. It is NOT standard for primary debulking or recurrent disease outside trials.
