NICE says not to offer brachytherapy alone to CPG 4 or 5 disease. It may, however, be considered with external-beam radiotherapy for CPG 2 to 5 disease. MRI confinement or short-course ADT does not convert monotherapy into a recommended option, and brachytherapy is not absolutely excluded as a boost.
