Extramural vascular invasion (EMVI) detected on MRI is a strong predictor of distant metastatic recurrence and overall survival in rectal cancer. EMVI-positive tumours have approximately 2-3× higher risk of developing distant metastases. It is one of the key factors (alongside T stage, N stage, CRM threat) guiding the intensity of neoadjuvant treatment: EMVI-positive tumours are often treated with more intensive neoadjuvant regimens (TNT) and/or longer intervals before surgery to achieve tumour regression.
