Asymptomatic isolated amylase/lipase elevation is common with ICI (10-15% of patients) and does not necessarily indicate clinical pancreatitis. Per ESMO/ASCO guidelines, asymptomatic grade 3 amylase/lipase without abdominal symptoms, imaging changes or organ dysfunction can be monitored while continuing ICI. This is an important exception to the general rule of holding ICI for grade 3 laboratory abnormalities. Symptomatic pancreatitis (pain, vomiting) does require ICI hold and corticosteroids.
