Adult granulosa cell tumour (GCT) is a sex cord-stromal tumour that secretes oestrogen and inhibin B (tumour markers). Standard surgical management for patients who have completed childbearing is TAH-BSO with comprehensive staging. GCTs have an indolent but persistent course — late recurrences (>10 years) are common. Inhibin B is the most sensitive tumour marker for surveillance. Adjuvant chemotherapy (BEP or carboplatin-paclitaxel) is considered for stage IC and above. Hormonal therapy (aromatase inhibitors) has activity in recurrent disease.
