Prescribing considerations
- Confirm the licensed indication and individual biochemical requirement.
- Account for the product's substantial sodium content and potassium content in renal impairment, heart failure, hypertension, pre-eclampsia and controlled electrolyte diets.
- In hypercalcaemia with renal impairment and hyperphosphataemia, the principal effect may be intestinal calcium binding rather than a meaningful rise in serum phosphate.
- Use careful surveillance during prolonged treatment because soft-tissue calcification and nephrocalcinosis are recognised phosphate-related concerns.
Contraindications and cautions
- Hypersensitivity to sodium acid phosphate or an excipient.
- Avoid in rare hereditary fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency because the preparation contains sucrose.
Monitoring
- Assess serum phosphate and calcium response.
- Review renal function and clinically relevant electrolytes, especially where sodium or potassium handling is impaired.
- During prolonged treatment, assess for biochemical or clinical evidence of calcification or nephrocalcinosis.
- Review gastrointestinal tolerability.
Clinical pharmacology
Phosphate is absorbed from the gastrointestinal tract, filtered and partly reabsorbed by the kidneys, with absorbed phosphate ultimately eliminated mainly in urine. Oral inorganic phosphate lowers serum calcium in hypercalcaemia and supplies phosphate in deficiency states.
Formulation and product differences
- The selected product is an oral effervescent tablet and must be dissolved in water.
- The formulation contains sodium bicarbonate, potassium bicarbonate and sucrose, so its electrolyte and excipient burden must be considered separately from its phosphate content.
