factor IX: clinical use

Prescribing considerations

• Treatment should be supervised by a clinician experienced in haemophilia.
• Assess individual recovery and clinical response because pharmacokinetics vary between patients.
• Consider thrombotic risk, cardiovascular or liver disease, recent surgery and central venous access complications.
• Maintain batch traceability and consider hepatitis A and B vaccination for regularly treated patients.

Contraindications and cautions

• Hypersensitivity to human coagulation factor IX or any excipient.

Monitoring

• Plasma factor IX activity and clinical haemostatic response, particularly during major surgery.
• Neutralising inhibitors when response is inadequate and after repeated exposure as clinically appropriate.
• Hypersensitivity during administration, especially with early exposures or suspected inhibitors.
• Record the product name and batch number after each administration.
• Account for reagent and reference-standard effects when interpreting one-stage clotting assays.

Clinical pharmacology

Replenine-VF is a high-purity, plasma-derived, vitamin K-dependent coagulation factor. Replacement temporarily corrects the deficient clotting pathway; recovery and clearance vary between patients.

Formulation and product differences

• Powder with supplied water for injections, reconstituted before intravenous administration.
• Full-volume and clinician-directed half-volume reconstitution produce different concentrations.
• Contains sodium and traces of human proteins other than factor IX.
• Use only supplied or approved transfer and administration equipment because factor IX may adsorb to unsuitable surfaces.
