What is epoetin alfa used for?
Licensed uses represented by the selected product include:
Symptomatic anaemia associated with chronic renal failure in adults and specified children receiving haemodialysis, and adults receiving peritoneal dialysis.
Severe symptomatic renal anaemia in adults with renal insufficiency who are not yet receiving dialysis.
Anaemia in adults receiving chemotherapy for solid tumours, malignant lymphoma or multiple myeloma who are considered at risk of transfusion.
Increasing blood collected from moderately anaemic adults participating in autologous blood predonation before major elective surgery.
Reducing exposure to donated blood in selected non-iron-deficient adults undergoing major elective orthopaedic surgery.
Symptomatic anaemia in selected adults with low- or intermediate-1-risk primary myelodysplastic syndromes and low endogenous erythropoietin.
How does epoetin alfa work?
Epoetin alfa binds to erythropoietin receptors on developing red blood cells in bone marrow, promoting their survival and growth. This increases reticulocytes, red blood cells and haemoglobin. Adequate iron and correction of other causes of anaemia support the response.

How to take epoetin alfa
Use only as instructed by the specialist team. It may be injected under the skin or into a vein. Self-injection requires appropriate training.
Let the pre-filled syringe reach room temperature naturally before use.
Do not shake, freeze or mix it with other injection solutions.
Check that the solution is clear and colourless, without particles, and that the syringe is undamaged.
For under-the-skin injection, suitable areas include the thigh or abdomen; rotate injection sites.
Each pre-filled syringe is for single use. Dispose of it in an approved sharps container.

Common or expected effects
- Nausea, vomiting or diarrhoea
- Fever or flu-like symptoms
- Headache or increased blood pressure
- Cough
- Rash
- Muscle, joint, bone or limb pain
- Injection-site redness, burning or pain
- Swelling of the hands, feet or ankles
Get urgent medical advice
- Blood clots, potentially causing chest pain, breathlessness, stroke-like symptoms or painful limb swelling
- Severe hypertension, sometimes with sudden severe headache, confusion or seizures
- Severe allergic reaction with facial or throat swelling or breathing difficulty
- Severe blistering or peeling skin reactions, including Stevens–Johnson syndrome and toxic epidermal necrolysis
- Pure red cell aplasia, causing sudden loss of response with worsening tiredness, dizziness or breathlessness
epoetin alfa in pregnancy
Evidence during pregnancy is limited, and animal studies found reproductive toxicity at high exposure. Treatment should be considered only when the expected benefit outweighs potential fetal risk, with specialist oversight. It is not recommended for pregnant surgical patients participating in autologous blood predonation.
epoetin alfa while breastfeeding
It is unknown whether injected epoetin alfa passes into human milk. The prescriber should weigh the benefits of breastfeeding against the clinical need for treatment. It is not recommended for breastfeeding surgical patients participating in autologous blood predonation.
epoetin alfa interactions
Few specific interactions are documented, but some treatments can affect response or monitoring.
Cyclosporin
Concentrations may change as the red blood cell proportion rises, so blood-level monitoring and adjustment may be needed.
Medicines that suppress erythropoiesis
These may reduce the response to epoetin alfa.
Iron supplements

Is epoetin alfa the same as erythropoietin?
It is a recombinant version of human erythropoietin and acts on the same receptor to stimulate red blood cell production.
How quickly does epoetin alfa start working?
Immature red blood cells may rise before haemoglobin. A measurable haemoglobin response commonly takes several weeks and varies with iron stores and illness.
Can epoetin alfa replace a blood transfusion?
It can reduce transfusion requirements in certain licensed settings but does not remove the need for transfusion in every patient. Transfusion may be preferred in some cancer-related situations.
