Prescribing considerations
- Confirm eligibility against current official recommendations and use the age-appropriate presentation.
- Postpone vaccination during acute severe febrile illness or acute infection; minor infection or low-grade fever alone need not delay it.
- Assess allergy history, bleeding risk, anticoagulant treatment and susceptibility to injection-related syncope.
- Immunocompromised people may have a weaker response; coordinate timing with specialist treatment plans where relevant.
Contraindications and cautions
- Hypersensitivity to cemivameran or any excipient, including its lipid components.
- Do not give a further Comirnaty vaccination after anaphylaxis to a previous Comirnaty vaccination without appropriate specialist assessment.
Monitoring
- Record the product name and batch number for traceability.
- Ensure treatment and supervision for anaphylaxis are immediately available.
- Counsel about myocarditis and pericarditis symptoms and the need for prompt assessment.
- Report suspected adverse reactions through the Yellow Card scheme.
Clinical pharmacology
Cemivameran is lipid-nanoparticle-formulated, nucleoside-modified mRNA encoding the prefusion-stabilised SARS-CoV-2 Omicron KP.2 spike protein. Expression is transient and induces antibody and cellular responses; conventional pharmacokinetic assessment is not applicable.
Formulation and product differences
- The selected presentation is a refrigerated, single-use glass pre-filled syringe supplied without a needle.
- It contains a white to off-white dispersion that is ready to use and must not be diluted, shaken, frozen or mixed with another medicinal product.
- The formulation includes the lipid components ALC-0315 and polyethylene-glycol-containing ALC-0159.

Cemivameran interactions
Cemivameran has no conventional metabolic medicine interactions, but some treatments may affect administration or the immune response.
Immunosuppressants
The immune response may be reduced; a specialist may help coordinate vaccination with current or planned treatment.
Oral anticoagulants
Intramuscular injection may cause bruising or bleeding. The vaccinator should assess anticoagulation stability and use appropriate injection technique.
