For bupivacaine with adrenaline, aspirate carefully and repeat before the full dose; for epidurals, give a 3–5 ml test dose with verbal contact and frequent heart-rate checks, preferably ECG, for five minutes afterward; inject the main dose slowly in increments at 25–50 mg/min with constant patient contact.
