Clozapine can rarely cause myocarditis, particularly early in treatment. Chest pain, shortness of breath, fever and fatigue require urgent medical assessment even if neutrophil monitoring is acceptable. Blood count monitoring does not rule out myocarditis. Delaying review could be dangerous.
