Activated macrophages within granulomas express unregulated extrarenal 1-alpha-hydroxylase, converting 25-hydroxyvitamin D to calcitriol and increasing intestinal calcium absorption. Vitamin-D supplementation can worsen hypercalcaemia; significant disease is treated by controlling sarcoid inflammation, usually with corticosteroid.
