Adhesive capsulitis causes painful global restriction of active and passive shoulder movement, classically external rotation, and is associated with diabetes. Rotator cuff tears mainly impair active movement and strength while passive movement is relatively preserved. Normal X-ray helps exclude glenohumeral OA.
