Ticagrelor can cause dyspnoea, often early after initiation, but stopping antiplatelet therapy after ACS can be dangerous. The current observations do not suggest acute anaphylaxis or severe respiratory compromise. The pharmacist should encourage adherence, safety-net for worsening symptoms, and arrange clinical review if the breathlessness is significant or persistent.
