From 22 January 2026 the MHRA removed the mandatory second-prescriber agreement for patients under 18. That change did not remove specialist initiation or the broader risk-minimisation programme. Updated acknowledgement materials, the required patient information video and existing counselling and monitoring of mental health, sexual function and pregnancy risk continue. A signature alone does not permit unrestricted GP initiation, and pharmacists cannot initiate treatment solely because screening tests are satisfactory.
