Linezolid is a reversible, non-selective MAOI. It can markedly potentiate the pressor response to sympathomimetics such as pseudoephedrine, a particular concern in poorly controlled hypertension. Dose separation does not remove enzyme inhibition, and delayed monitoring misses the acute risk. A reduced OTC dose is not an adequate safeguard. The pharmacist should avoid the sympathomimetic, offer a suitable non-sympathomimetic measure if appropriate and communicate with the treating team; an OTC antibiotic cannot replace linezolid.
