This barefoot farmer walking on red laterite (volcanic) soil is exposed to colloidal silicate and aluminium microparticles that penetrate the interdigital skin of the feet, are taken up by macrophages in the dermal lymphatics, and provoke a chronic granulomatous endolymphangitis. The finding of non-caseating granulomas containing birefringent (mineral) particles on biopsy is the classic early histological picture before lymphatic obliteration produces the mossy, elephantiasic swelling of established podoconiosis. Geography (East African highlands), occupation (barefoot subsistence farming) and irritant red clay soil are the key discriminators pointing away from infective or neoplastic causes. Why the other options are wrong: C. Kaposi sarcoma: presents as violaceous or dark vascular nodules with spindle cell proliferation and vascular slits on histology, not granulomas with birefringent particles, and is HHV8 driven rather than soil related. A. Foreign body granuloma from thorns: causes localised granulomas around an organic foreign body at a puncture site, not multiple bilateral sole nodules linked to chronic soil exposure with characteristic mineral birefringence and lymphatic pathology. E. Melanoma: would show atypical melanocytes with pigment, not granulomatous inflammation, and does not relate to soil exposure. D.
