Eplerenone must not be started when serum potassium is above 5.0 mmol/L. The ACE inhibitor adds further hyperkalaemia risk, although ACE inhibitor plus eplerenone is not itself the prohibited triple combination of ACE inhibitor, ARB and eplerenone. The pharmacist should stop initiation and contact the prescriber; after a safe start, potassium is checked within one week and at one month.
