Regulation 5 requires the pharmacy record to include, for an absence, its date and the times the absence commenced and ended. The record separately identifies the responsible pharmacist and when responsibility began and ceased. It does not require a personal reason, staff list, prescription log or business-owner countersignature as part of the statutory absence entry.
