SGLT2 inhibitors should usually be withheld during acute illness with dehydration or reduced intake because of ketoacidosis risk. Patients should be counselled about symptoms such as nausea, abdominal pain, deep breathing and drowsiness. Insulin should not be stopped without specialist advice. Restarting should occur once the patient is eating and drinking normally and clinically stable.
