The symptoms are consistent with possible lithium toxicity, which can be precipitated by dehydration and NSAIDs such as ibuprofen. Urgent lithium level, renal function and clinical assessment are needed. Continuing lithium may worsen toxicity. The NSAID should be flagged and alternative analgesia considered.
