Bisphosphonates such as alendronic acid have very poor oral bioavailability, which is further reduced by food, calcium, and other minerals. They must be taken first thing in the morning on an empty stomach with a full glass of plain water (not mineral water, tea, coffee, or juice). The patient should remain upright (sitting or standing) for at least 30 minutes after taking the dose and before eating, to reduce the risk of oesophageal irritation and ulceration.
