Phosphodiesterase-5 inhibitors can markedly potentiate the hypotensive effect of nitrates. Sildenafil must not be used while regular or rescue nitrate treatment may be required; an arbitrary dose interval or dose reduction does not make the combination safe. The patient needs prescriber review of erectile-dysfunction and anti-anginal management rather than over-the-counter supply.
