CD register discrepancies must be thoroughly investigated and documented. The pharmacist should check for possible explanations (dispensing errors, breakages, incorrect entries). If the discrepancy cannot be accounted for, it should be reported to the responsible pharmacist and the CD Accountable Officer. Simply adjusting the register without investigation is unacceptable. The register must not be altered to disguise discrepancies. Contacting the police is premature until an investigation has been conducted.
