Broad-spectrum antibiotics such as co-amoxiclav can potentiate the effect of warfarin by disrupting gut flora that synthesise vitamin K, leading to a raised INR and increased bleeding risk. The BNF advises that INR should be monitored more closely when antibiotics are started or stopped in patients on warfarin. Empirically reducing the warfarin dose is not recommended without INR guidance.
