For Schedule 2 and 3 Controlled Drugs (except temazepam), the prescription must include: the patient's name and address, the prescriber's signature and date, the drug name, form, strength, dose, total quantity in words and figures, and directions. Omitting the form (e.g. tablets, capsules, modified-release tablets) renders the prescription legally incomplete. The prescription is valid for 28 days from the date, so 10 days is within the valid period. Handwritten prescriptions for CDs remain legally valid.
