Correct answer: AWithhold to grade 1 or baseline; resume at 240, 180 or 120 mg daily

For grade 3 or greater or intolerable toxicity, apalutamide is withheld until symptoms improve to grade 1 or baseline. It can then resume at the same dose or a reduced 180 mg or 120 mg dose. Continuing through grade 3, inventing a 60 mg dose, or mandatory permanent cessation after the first event does not follow the SmPC.
