The Lynparza SmPC says to interrupt treatment and investigate severe haematological toxicity or transfusion dependence. If blood parameters remain clinically abnormal after 4 weeks, bone-marrow analysis and/or blood cytogenetic analysis are recommended. Suspected MDS or AML requires haematology referral, and confirmed MDS or AML requires permanent olaparib discontinuation and appropriate treatment. Persistent multilineage cytopenia is not safely managed by automatic rechallenge, class switching or isolated growth-factor support.
