The Calvert formula is total dose in mg = target AUC × (GFR + 25). Thus 5 × (52 + 25) = 5 × 77 = 385 mg. The result is not expressed per square metre. Subtracting 20%, omitting the non-renal 25 term or replacing the measured GFR with an arbitrary minimum would each produce an incorrect exposure.
