Internal inflammatory root resorption begins on the pulpal surface of dentine and typically produces a smooth, symmetrical or balloon-like enlargement of the pulp chamber or root canal. The defect remains centred on the canal when the horizontal angulation is changed. An eccentric, irregular defect that moves relative to the canal on parallax views favours external root resorption (B). A lesion centred beyond the apex suggests apical periodontitis or another periapical lesion (C), while a radiopaque apical mass is not characteristic of inflammatory internal resorption (D).
