Lactobacilli are classically associated with progression of an established carious lesion into dentine. Their strong acid production and ability to tolerate a low pH allow them to thrive in the acidic, retentive environment of a cavitated lesion. They are therefore regarded mainly as secondary contributors or enhancers of lesion progression rather than the principal initiators of enamel caries. Initial enamel demineralisation is produced by a dysbiotic cariogenic biofilm and has traditionally been associated more strongly with mutans streptococci. Lactobacilli do not promote lesion arrest or maintain a health-associated biofilm. Although they may be detected in root caries, initial colonisation of a newly exposed root surface is not their characteristic role.
