Midazolam is a benzodiazepine and a positive allosteric modulator of the GABA-A receptor. It binds at a benzodiazepine site distinct from the GABA-binding site and enhances the effect of GABA on the receptor-associated chloride channel. This increases inhibitory neurotransmission, producing anxiolysis, sedation and anterograde amnesia. Midazolam does not directly activate the receptor in the absence of GABA. NMDA receptors are principally associated with drugs such as ketamine. Opioid receptors mediate the effects of agents such as fentanyl and remifentanil, rather than midazolam. Serotonin and dopamine receptors are not the principal molecular targets responsible for midazolam sedation.
