In pattern analysis dermoscopy, melanocytic lesions are classified by their global pattern. Benign naevi typically show simple/organised patterns: reticular (pigment network), globular, homogeneous, or starburst (Spitz/Reed naevus). A multicomponent pattern — containing 3 or more distinct dermoscopic structures (irregular network, irregular dots/globules, blue-white veil, regression structures, irregular blotches, atypical vessels) — is the pattern most suspicious for melanoma and warrants excision biopsy. The principle is that melanoma, due to its heterogeneous biology, produces multiple different dermoscopic structures reflecting different histological components, whereas benign naevi are architecturally uniform.
