Dupilumab is not licensed for use in pregnancy (category B — no adequate human studies), but animal reproductive studies have not demonstrated teratogenicity. Real-world pregnancy registry data and case series have not identified safety signals to date. BAD/NICE guidelines recommend discussing risks and benefits with the patient and making individualised decisions. For women with severe eczema where Dupilumab has been the only effective therapy, continuing may be considered after careful counselling. Azathioprine remains the systemic with the most established pregnancy safety data. The decision should be made jointly with the patient, dermatologist, and obstetrician.
