Abrocitinib is an oral selective JAK1 inhibitor approved for moderate-to-severe atopic eczema (NICE TA867). JAK1 mediates signalling of multiple pro-inflammatory cytokines relevant to eczema pathogenesis, including IL-4, IL-13, IL-31, and thymic stromal lymphopoietin (TSLP). Selectivity for JAK1 over JAK2 aims to reduce haematological side effects. Important safety considerations include VTE risk, herpes zoster reactivation, and hyperlipidaemia. NICE recommends JAK inhibitors as an option when conventional systemic therapies have failed and biologics are inappropriate or ineffective.
