BAD guidelines recommend excision margins based on Breslow thickness: melanoma in situ 5-10 mm; pT1 (≤1.0 mm) 1 cm; pT2 (1.01-2.00 mm) 1-2 cm; pT3 (2.01-4.00 mm) 1-2 cm; pT4 (>4.0 mm) 2 cm. For a 1.8 mm melanoma (pT2), the recommended margin is 1-2 cm. Sentinel lymph node biopsy should also be discussed for melanomas ≥0.8 mm with high-risk features or ≥1.0 mm.
