NICE 2022 gout guidelines recommend anti-inflammatory prophylaxis during ULT initiation to prevent mobilisation flares. Colchicine 0.5 mg once or twice daily is the preferred prophylactic agent. Prophylaxis should continue for up to 6 months after the serum urate target has been achieved (not from ULT start date). Since dose titration may take several months total prophylaxis duration can be 9-12 months or longer. Low-dose NSAIDs or low-dose Prednisolone are alternatives if Colchicine is contraindicated. Adequate prophylaxis duration significantly reduces flare frequency during ULT initiation and improves patient confidence and treatment adherence.
