Voclosporin has a sufficiently predictable exposure-response profile to permit standard dosing without routine measurement of trough drug concentrations. This contrasts with systemic tacrolimus, whose variable pharmacokinetics and narrow therapeutic index ordinarily require trough-guided individualisation. This convenience must not be confused with an absence of monitoring: the voclosporin SmPC requires baseline and serial eGFR assessment, blood-pressure monitoring and dose modification for renal deterioration or relevant CYP3A4 interactions. Both drugs inhibit calcineurin, so option E is incorrect, and voclosporin can cause haemodynamic eGFR reduction, hypertension and hyperkalaemia, excluding option B.
