EULAR 2022 recommends a treat-to-target strategy aiming for remission (DAS28 <2.6 or Boolean remission) as the primary target. If remission is not achievable low disease activity (DAS28 <3.2) is an acceptable alternative target. Treatment should be adjusted every 3 months until the target is reached. This patient with DAS28 3.8 (moderate disease activity) has not achieved target and treatment escalation should be considered (adding another csDMARD or switching to subcutaneous Methotrexate before biologic escalation).
