The correct answer is 24 weeks. NICE TA752 permits continuation of belimumab beyond 24 weeks only when the SELENA-SLEDAI score has improved by at least 4 points. This is consistent with the current SmPC, which advises continuous assessment and consideration of discontinuation when disease control has not improved after 6 months. Eight weeks is too early for the formal UK continuation decision. Although efficacy outcomes have been reported at 52 weeks and longer follow-up points, these are not the NICE-mandated assessment point. The original wording about “maximal response” was imprecise: belimumab has no single established time at which maximal clinical effect is reached, and improvement may continue beyond 24 weeks.
