EULAR 2023 recommends Hydroxychloroquine for ALL patients with SLE at a target dose of 5 mg/kg real body weight/day, regardless of disease severity. HCQ reduces flare frequency, damage accrual, and thrombotic risk. It also improves survival. Retinal monitoring is required (baseline assessment, then annual screening from year 5 per RCOphth). HCQ should be continued during pregnancy. Methotrexate and Azathioprine are used for more active disease. Biologics are reserved for refractory cases.
