vonicog alfa: clinical use

Prescribing considerations

• Confirm that desmopressin alone is ineffective or contraindicated.
• Use under specialist haemostasis supervision and individualise management using bleeding severity, clinical response and laboratory results.
• Assess baseline factor VIII activity and whether separate recombinant factor VIII is required for rapid haemostasis.
• Consider sodium intake, polysorbate allergy and previous exposure to plasma-derived von Willebrand factor.
• Record the product name and batch number for traceability.

Contraindications and cautions

• Hypersensitivity to vonicog alfa or any excipient.
• Known allergic reaction to mouse or hamster proteins.

Monitoring

• Clinical haemostasis and breakthrough bleeding.
• Von Willebrand factor activity and factor VIII activity, particularly around surgery or combined factor replacement.
• Immediate hypersensitivity during infusion.
• Early evidence of thrombosis in patients with risk factors.
• Test for von Willebrand factor inhibitors if expected recovery is not achieved or bleeding remains uncontrolled.

Clinical pharmacology

Vonicog alfa is recombinant human von Willebrand factor containing the range of plasma multimers plus ultra-large multimers. It restores platelet adhesion and stabilises endogenous factor VIII, producing an immediate primary-haemostasis effect and a later rise in factor VIII activity.

Formulation and product differences

• The licensed presentations differ in vial content and supplied solvent volume but produce the same reconstituted concentration.
• Each pack contains lyophilised powder, water for injections and a Mix2Vial reconstitution device.
• The product contains trace recombinant factor VIII, mouse immunoglobulin G and hamster proteins.
