Prescribing considerations
- Treatment should be supervised by a clinician experienced in alpha-mannosidosis or lysosomal enzyme replacement therapy.
- Ensure immediate access to management of hypersensitivity, anaphylaxis and other infusion-related reactions.
- Consider home infusion only after satisfactory tolerance, individual assessment and patient or caregiver training.
- Periodically evaluate clinical benefit; consider discontinuation if no clear benefit is observed.
Contraindications and cautions
- Severe allergic reaction to velmanase alfa or any formulation excipient.
- Use caution after infusion-related or hypersensitivity reactions; management may require interruption, slower administration or supportive treatment.
- Established neurological, cognitive and irreversible skeletal complications are not expected to respond because the enzyme does not cross the blood–brain barrier.
Monitoring
- Record product name and batch number for biological traceability.
- Observe for infusion-related reactions during administration and afterwards according to clinical circumstances.
- Assess ongoing clinical benefit and deterioration.
- Test for anti-velmanase alfa antibodies after severe infusion reactions or loss of treatment effect.
Clinical pharmacology
Velmanase alfa is recombinant human lysosomal alpha-mannosidase. It is taken up by cells through mannose receptors and degrades mannose-rich oligosaccharides within lysosomes.
Formulation and product differences
- White to off-white powder requiring aseptic reconstitution for intravenous infusion.
- The reconstituted solution should be clear; do not use if discoloured or containing opaque particles.
- Administration requires a pump and low protein-binding in-line filter. It must not be mixed with other medicines.
