Prescribing considerations

• Treatment should be supervised by a clinician experienced in Gaucher disease.
• Ensure emergency support is available because infusion-related and anaphylactic reactions can occur.
• Home administration requires careful patient selection, training and continuing specialist oversight.
• Record the product name and batch number to maintain biological-medicine traceability.

Contraindications and cautions

• Severe allergic reaction to velaglucerase alfa or any excipient.
• Use caution after hypersensitivity to velaglucerase alfa or another enzyme replacement therapy.
• Safety and efficacy have not been established in children younger than 4 years.

Monitoring

• Observe for infusion-related or hypersensitivity reactions during and after administration.
• Follow clinical manifestations and agreed therapeutic goals, such as haematological and organ-related response.
• Consider anti-velaglucerase alfa antibody testing if an immune-mediated reaction or unexplained loss of effect is suspected.
• Monitor Gaucher disease closely during pregnancy and the puerperium.

Clinical pharmacology

A recombinant glycoprotein with the same amino-acid sequence as human glucocerebrosidase. Predominantly high-mannose glycans promote macrophage uptake through mannose receptors, delivering enzyme activity to lysosomes; circulating protein is cleared rapidly.

Formulation and product differences

• The selected product is a white to off-white powder for reconstitution and dilution before intravenous infusion.
• Each vial contains 400 Units; the excipients are sucrose, sodium citrate dihydrate, citric acid monohydrate and polysorbate 20.
• The vial is single use, requires aseptic preparation and must not be mixed with other medicinal products.
• Store refrigerated, do not freeze and protect from light.
