Prescribing considerations
- Confirm symptoms are consistent with hardened wax rather than infection, perforation or another ear disorder.
- Ask about previous ear disease, surgery, recent syringing and attempted mechanical wax removal.
- Advise against cotton buds or other objects, which may impact wax or injure the canal.
Contraindications and cautions
- Known or suspected damaged eardrum.
- Dizziness or current ear pain, discharge, inflammation, infection or tinnitus.
- History of ear problems unless used under close clinical supervision.
- Recent ear syringing or canal trauma from attempted mechanical wax removal.
- Hypersensitivity to urea hydrogen peroxide or an excipient.
Monitoring
- Stop if pain or irritation develops.
- Reassess persistent or worsening hearing loss, dizziness, tinnitus or discomfort.
- Check for discharge, inflammation or canal injury before recommending further drops.
Clinical pharmacology
Acts locally in the external auditory canal. Oxygen release disrupts wax; hydrogen peroxide is cerumenolytic, while glycerol and urea soften debris and urea provides mild keratolysis. Systemic pharmacokinetic data are not described.
Formulation and product differences
- The selected product is a clear, straw-coloured, viscous ear-drop preparation containing glycerol and 8-hydroxyquinoline as excipients.
- Presentations may use a glass bottle with pipette applicator or a plastic bottle with nozzle; not all pack sizes may be marketed.
