Prescribing considerations
- Diagnostic use only; restricted to authorised personnel in designated nuclear-medicine settings.
- Justify radiation exposure individually and use the lowest activity providing the required diagnostic information.
- Review medicines affecting myocardial function or blood flow when interpreting images.
- Not recommended for children or adolescents because supporting data are unavailable.
- Use caution in renal or hepatic impairment because pharmacokinetics are uncharacterised and radiation exposure may increase.
Contraindications and cautions

Monitoring
- Confirm pregnancy and breastfeeding status before administration.
- Ensure facilities and trained staff for managing anaphylaxis are immediately available.
- Encourage hydration and frequent voiding afterwards.
- During stress testing, monitor according to the selected stress procedure.
- Interpret breast imaging cautiously: small lesions may be missed, negative imaging does not exclude malignancy and axillary staging is unsupported.
Clinical pharmacology
Technetium-99m tetrofosmin is a lipophilic diagnostic complex whose myocardial uptake relates to coronary blood flow. Uptake is rapid and sufficiently retained for planar or SPECT imaging; background activity clears through urinary and faecal routes.
Formulation and product differences
- The product is a non-radioactive powder kit requiring reconstitution with sodium pertechnetate technetium-99m.
- Vial contents must not be injected directly. The prepared product is a clear, colourless intravenous solution requiring radiochemical-purity testing and radiation-safe handling.
tetrofosmin interactions
Formal interaction studies have not been performed. Medicines affecting heart function or coronary blood flow can alter scan accuracy.
Beta blockers
May reduce detection of coronary artery disease, potentially contributing to a false-negative result.
Calcium-channel blockers
May change myocardial function or blood flow and affect scan interpretation.
Nitrates
May alter coronary blood flow and reduce the visibility of perfusion abnormalities.
